APPLICATION FORMS

Please click your
desired form and save it to your
computer. Then use your printer to print
the form that you just saved. Follow the
directions on the form. Please present
this completed form to the Chapter
Coordinator for validation. A validated
copy, signed by the Chapter Coordinator
or designee, is necessary for admission
to chapter sessions of other pools. (Please
note that dial-up internet connections
may take a little longer to display the
forms.)

If you are not able to view and/or
cannot print one of the above forms,
then please check the following:
Q. Did the form display on your screen
when you clicked on one of the above
choices?
A. Your computer is already been
configured to properly display Adobe
files on your screen so there is no need
to save the file first. Simply click
the Printer Icon displayed in the Adobe
window.
Q. Are you having problems printing the
file you saved?
A. Chances are that you do not have the
Adobe Reader installed on your computer.
